Illinois Just Ran Its First Year as Its Own ACA Marketplace — Here's What Happened

For 2026 coverage, Illinois stopped using HealthCare.gov and began running its own ACA marketplace, Get Covered Illinois, for the first time. The debut year came with new state authority to extend deadlines, a heavily funded local-help network, and — by late spring — a real enrollment decline driven mostly by nonpayment as costs rose. Here's what the state's own numbers show. Illinois Now Runs Its Own ACA Marketplace

Since November 1, 2025, Illinoisans have applied for and enrolled in 2026 coverage through Get Covered Illinois (GetCoveredIllinois.gov) instead of HealthCare.gov. The switch follows a 2023 state law directing Illinois to move off the federal platform, and 2026 is the state's first plan year running full marketplace operations — enrollment, eligibility, and plan comparison — on its own system rather than the federal government's.

Its First Deadline Extension

Open enrollment for 2026 coverage was originally set to close January 15, 2026. On January 12, Get Covered Illinois announced a 16-day extension, pushing the deadline to January 31 for coverage effective February 1. "As a state-based marketplace, not only do we now have the authority to offer this special enrollment extension, but we also have more resources to support our customers in finding affordable health coverage options," said Director Morgan Winters — the first such extension since the state-run marketplace launched.

Nearly $7M for Local Enrollment Help

Illinois has invested nearly $7 million in navigator grants, funding a program of more than 140 navigators statewide. Separately, the state has also built a certified network of about 9,000 licensed brokers who help residents shop for and enroll in coverage. Both navigators and brokers offer enrollment help at no cost to the consumer.

Enrollment Has Slipped From Its Record High

Illinois' marketplace enrollment stood at 373,065 as of May 31, 2026 — down nearly 15% from the state's record high of 437,892 in February 2025. Of the roughly 92,000 people who lost coverage over that stretch, about 64% were disenrolled specifically for nonpayment as of May 31 — the highest share since at least 2020, according to state data. The decline has been linked in state and independent reporting to the expiration of enhanced federal premium tax credits, which pushed monthly premiums higher for many enrollees.
